Adoption and Sterlization Contract

I agree to the following conditions for adoption of the above described animal:

1. To provide food, water, shelter, humane treatment, and veterinary care for the animal throughout its lifetime.

2. To have the animal vaccinated against Rabies at 6 months of age and every year thereafer as required by law.

3. To purchase a county dog tag each year and to attach the tag to the collar which is to be worn at all times. This is required by law. (Cats Exempt)

4. Not to sell, trade, give away, or allow the animal to be used for experimental research, or to be used for breeding purposes. If I can no longer keep the animal, it must be returned to the Animal Shelter.

5. I have paid a fee of $_____ (call shelter for fee amount) as FULL payment for the sterilization of the animal.

6. I understand this payment arrangement is only valid at Jackson Animal Clinic or Pocatalico Animal Hospital.

7. I agree to allow an agent from the Jackson County Animal Shelter to remove this pet from any premises occupied by me if these conditions are not met; entry shall not contstitute a trespass.

8. I agree to have this animal spayed/neutered by _____________.
