I am currently following "The New Rules of Lifting" by Lou Shuler and Alwyn Cosgrove. It is a great book. There are four different scenarios that fit people's lifstyles and with these scenarios are templates for a year's worth of workouts. Nutrition is not a huge issue in this book. However, with some reading and help, a good nutrition plan can be worked out.
I am currently in week 19 in this program. I have done two fat loss programs and currently in the middle of a hypertrophy program. There are built in cardio sessions and break weeks within the program. A very well designed program.
