All phones in Italy are able to make both Domestic and Long Distance calls. 

Phone cards (EuroStar is best) are available at Tobacco shops and bars.  To make an international call:

1) Insert the card
2) wait for a second dial tone
3) dial the country code (01 for USA) area code and number.

If you prefer to charge the call to your credit card, the AT&T Direct access number from Italy is 800-172-444.  To make a call

1) Dial the access number
2) Dial the country code, area code and number
3) Dial your credit or calling card number

You may be able to find better phone rates by shopping around for a calling card before you go.
