| Before your child comes to my home there are a few things that I have parents complete. This way there are never any questions or misunderstanding. At the same time, I encourage the Parents to bring their child to my home to explore, and familiarize themselves with their surroundings. Putting your child into child care can be traumatic to Parents and their child, so I want to make sure the transition is as smooth as possible. Caregiver/Parent Agreement; This is a contract signed by the Parent(s) and myself and includes the agreed upon child care fees, the Policies of the service, and a brief discription of what my expectations are re; items I will provide vs. items the Parent(s) must supply. Child Information Sheet; An informatin sheet that covers the childs basic information, medical conditions, likes, dislikes, methods of dicipline, childs routine etc. Medication Consent form; This is a form signed by the Parent, listing any medication the child is taking, as well as consent to give certain over the counter medication ( eg. Tylenol for fever, Gravol for nausea and vomitting) should your child become ill while in my care. |

| A child care journal will also be provided by myself to use as a method of communication between myself and the Parent(s), the option or receiving this through email is also available. I will make an entry every day your child is in my care, briefly listing the childs day, as well as any questions, concerns or issues. Parents are encouraged to use the journal as well should they need to pass any information on to me. Again, just another way to avoid confusion and misunderstanding. Picking up your child after a long day at work can be pretty chaotic at times, and conversation can easily be forgotten when all you want to do is get home. | |||||||||||||||||||||||||
