WHAT TO BRING TO YOUR APPOINTMENT

  • Your insurance card
  • Physician referral forms if required by insurance
  • A list of current prescriptions and/or over-the-counter medications your are taking, including dose and frequency
  • All pertinent previous neurological evaluations and/or records
  • Any appropriate reports or MRI records you may have

We make every effort to run our practice on time. We would therefore appreciate it if you  could plan on arriving 15 minutes before your scheduled appointment time in order to allow for parking time.

INSURANCE AND PAYMENT INFORMATION

Make sure you bring your insurance card with you for each visit. Please advise the receptionist if you have a change of insurance or address. If your insurance requires a referral have your primary care physician issue one prior to your visit. We also recommend that your doctor include more than one visit on your referral.

Our office participates in most insurance plans. We will do our best to alert you if we are not in a particular plan; however, we recommend that a patient confirm with their carrier prior to making an appointment or scheduling a test. Copayments are expected to be paid at the time of the visit. A service fee will be attached to the bill if a copayment has to be billed. We will file all other plans as a courtesy.

We are part of the Medicare system and our services are covered. We will submit a claim for you however Medicare will mail a check directly to you for a portion of the bill. You are responsible for 100% of the bill at the time of visit.

Patients without appropriate pre-approved from their insurance carrier may have their visits cancelled or denied.

OFFICE POLICIES: TEST RESULTS, PRESCRIPTIONS, AND FEES 

Test Results: For results of lab work, CAT scans, MRI’s, EEG’s, and so forth, please wait at least three days after the last test before calling for the results. Approximately two weeks is needed before all the spinal tap results are available. Please be sure to call our office if you haven’t heard about your test results.

Prescriptions: All prescriptions written at the time of the visit should be taken to your pharmacy to be filled. Check your prescriptions before calling our office for a refill to avoid extra phone calls. Routine refill requests will be honored during regular office hours only. Be sure to advise your doctor or staff member when you have a mail-a-way pharmacy and/or require a 3 month supply of medication.

Fees: Appointment cancellation must be at lease 24 hours advance notice, otherwise a charge may be credited to your account. Requests for forms, letters, copies, of records, etc. will be quoted a fee for service. Payment is usually requested in advance.

PRINT PATIENT FORMS 

 You can view and/or print the necessary patient forms for your appointment by following these easy instructions. In order to view and/or print the forms you must have the Adobe Acrobat Reader™, which you probably already have on your computer. If not, you can easily and quickly download a fee version of the reader by going to their website: Download Reader.

When viewing the forms in the Adobe Reader, the text might appear somewhat fuzzy; this is normal. To print, just click the printer icon in the Adobe Reader toolbar above the document.
