REGISTRATION:  In order for Imaginations to reserve a space for your child, please complete this form and return it along with your child's non-refundable enrollment fees to the above address.
ENROLLMENT FEES:  This fee includes processing your child's application, art & school supplies, personalized Imaginations tote bag, and your child's accident insurance premium. A portion of it is also credited to your child's last week of care.
ENROLLMENT:  Prior to your child's first day at Imaginations, all enrollment information must be completed, signed and returned to Ms. Melanie with your child's tuition.
TUITION:  Your tuition payment reserves your child's space in our program. In order to insure quality programming and proper staffing, your tuition is due even when your child is absent.
