To apply:  Print out a copy of this application using your printer and send it to us.  When we recieve your registration we will send you the necessary software you need to print the forms and detailed information and instructions to get you started.  If your printer does not meet the quality expected we will send you a refund within 48 hours of reviewing your registration.  This form, printed on your printer, must arrive along with your payment of $5.00 or your request will be denied and returned immediately.

Unfortuantely, we must charge this one time fee to cover the cost of copying the software, diskettes, shipping and registration processing.  Please send the payment in a money order, or personal check form.  (Personal checks held 7 to 10 days).  The fee can be recouped (NOT REFUNDED).  Again, this is the ONLY fee you will have from us.  We will never ask for another cent from you!
