Thank you to the many teachers who posted their ideas online to benefit other educators.  Here are a few that I found especially creative. There are some amazing  links at the bottom as well.
1) Homework Buddies: Pair students to help one another with dismissal time. Homework buddies check to make sure that their buddy's homework assignments are written down completely and that all of the necessary books are in their book bag. Once complete, the students earn a sticker on a 5x5 grid incentive chart on their homework folder.  When the chart is full, students earn a prize.

Sunshine Club: To get students to walk in straight, quiet lines in the hallway, start a Sunshine Club.  If students misbehave in line, they join the club and must run 5 laps before playing at recess. This is healthful and helps keep those hallways quiet!

Quiet Lines: Sometimes students pretend to be "spies" in the hallway. They tip-toe past doorways, duck under windows and give a quiet "Whew!" when they reach their destination. (Wipe hands across forehead.)

10 Second Transitions: At the end of a lesson, tell students exactly what you would like them to do in order to prepare for the next class.  Then tell them, "You have 10 seconds."  Watch them scramble as you begin to count. Transition time will move along much more rapidly!

Team Banners: Group students and let them choose a team name. Design and hang team penants or banners above team groups to help teach and reinforce team responsibility.

Music Box Wind Up: On Monday, wind up a music box.  At each transition time, clearly explain what students are to do and then give them a moment to complete.  Then lift the lid to signal "return to seats." Also lift the lid if the class becomes rowdy. If there is still music left on Friday, allow students to have free time.
1) Give Me Five: Eyes on Speaker, Quiet, Be Still, Hands Free, Listen. Teacher raises hand and says "Give me Five!" Students raise their hand and follow 5 rules above.

Rainstick: To signal "quiet" in a relaxing way. 
1) Hula Hoop Groups - Place hula hoops around perimeter of room.  Groups sit around the outside of the hoop and keep their "activity" inside the hoop.  This works really well if rotating centers/groups.

Pencil Sharpener - Teacher places a green "go" circle by pencil sharpener signaling that the sharpener is open for use, "red" circle when it is not open for use. (I allow students to sharpen pencils at beginning of the day, after recess, and during the afternoon break time.) This prevents the "whirring" of the sharpener during instruction.

Bathroom Management -
Punch-Out Cards: students have cards with their name, teacher's name, and room number written on it. Numbers 1-5 are written at the bottom. Students may use the bathroom/get water at unscheduled times, but must punch a number out in order to go. When numbers 1-5 have all been punched out for the week, they may still go, but must miss 5 minutes of recess time in order to "go."

Cup System: Two plastic colored cups (one color for girls, one color for boys) sit by the door.  If a student needs to use the restroom, they may get their colored cup and place it on their desk.  This way only 1 girl and 1 boy are gone at the time and the teacher knows exactly which child is in the restroom.  A sign out/sign in sheet is also good so that the teacher can keep track of how frequently students need to be excused to monitor for potential problems.
