A) Full Name
B) Age - This is an adults only coven, you must be at least 18 years of age.
C) Basic background information including prior affiliation with any other spiritual path or organization, hobbies, and interests.
D) An explanation of why you are interested in joining the Coven.
Your application for membership will be reviewed and we will contact you to set up a meeting with members of the Coven.
