I require prescreening before making any overnight, out-of-area, or travel anywhere appointment. It will make any other appointment go much smoother as well. I will need your full name, a personal and business phone to verify, and a scan of your legal drivers license (front and back please) or other recognizeable ID that matches your name, sufficient for me to identify you when we meet.
