Our mission is a simple one.

To help most people in good health realize and appreciate what they have.

Also, to help a few in need of assistance to enhance their quality of life and to get a second chance.

Teresa Weers-Slaby contracted a rare virus that affected her heart in 1995. She was placed on the heart transplant list at UCLA in 2001. Due to medical complications she did not receive the needed transplant and passed away in February 2004.

In honor of Teresa’s courage through her 10-year struggle to survive, Michael Slaby, her husband and the entire Slaby and Weers families have created this trust fund in her honor, to raise funds for other patients that need a life saving heart transplant and for their ongoing monetary expenses to remain healthy. All contributions go directly to patients in the NTAF program. Thank your for contributing to this worthy cause and God Bless You!
