Rules and Regulations Governing the Use of Health Point Family Privilege Card

1. One card covers the entire family:

This privilege card is valid for one (1) year and renewable thereafter.

For Married members,  the following are considered  qualified dependents:

1. Legitimate spouse
2. Legitimate, illegitimate, legally adopted or stepchildren below 21 yrs old.
3. Children 21 yrs old and above but suffering from congenital disability or any disability acquired before the age of 21 that renders them totally dependent on the member for support.
4. Parents of members who are 60 years old or above.

      For Unmarried members, the following members of family are covered:

A.     Unmarried siblings below 21 yrs old

B.     Siblings 21 yrs old and above but suffering from congenital disability or any disability acquired before the age of 21 that renders them totally dependent on the member for support

C.     Parents who are 60 years old or above

D.     Illegitimate children

2. UNLIMITED free consult with the in house physicians and Dentist

The in house physician and dentist are  available during office hours, 8am – 5pm daily from Mondays to Saturdays.  All members needing subspecialty consults must first be evaluated by our in house physician and will be referred accordingly.

The in house physician will only render primary care and reserves the right to refer the patient to a subspecialist if deemed necessary.

3. 25% discount on all  subspecialty consults

Only NEW subspecialty consults will be covered by this provision.  Patients previously seen by our subspecialists prior to availment of this privilege card will not be covered unless they are referred to another subspecialists in which case the 25% discount will become applicable.
