| 1) Your completed and signed application to the address listed below. Please attach an additional paper if you need more room to list allergies, medications or medical history (A signature is required). 2) Enclose a copy of your most recent EKG which can be obtained from your physician. 3) A check or money order made payable to HealthFIT Services, or place your credit card information on the order form. |
| MEMBERSHIP BENEFITS |
| Annual Membership allows you to maintain complete and up to date medical records in a computer medical file. You are entitled to one free update per year to make changes to your medications, medical history, allergies, etc. This also allows you to purchase additional updates of your HEALTHFIT Card for the discounted rate of $10 per card. The first year's membership fee is waived with the purchase of a HEALTHFIT Card. Annual Membership is $15 per year. |
