Premier Service Agreement – 12 Month Term

Notify Quick and its client (_________________________________) agree to collaborate to provide phone announcement services as follows, 
effective _________________, 2002:

 SET UP PROCESS:

1) Client will provide a file of member phone numbers in list format (e.g.
Excel, ASCII, comma delimited). Lists can be sent to [email protected]. If an electronic file cannot be produced, the client may provide phone numbers on type-written paper; a data entry fee of $2.00 per page will be charged.

 2) Client prepays $1.50 per phone number up front; this covers up to 
10 call out sessions over a 12 month period. Additional calls beyond 
10 sessions will be billed 10 cents per number dialed.

 3) Client may include a second list of daytime phone numbers (in 
addition to the list of home phone numbers) for the same group of 
people, at no additional cost. This assumes that calls will only be 
placed to one of the two lists in a given situation.  If calls are placed 
to both home and daytime lists on the same day, client will be billed 
an additional 20 cents per number dialed for the second list.

 4) Notify Quick will add dialers and phone lines as needed to assure 
prompt calls for premier clients, even during widespread storms or 
times of heavy usage. Premier lists will begin dialing within 20 
minutes of client request. Calls are typically made at a rate of 
80 per minute, given a 12 second message.

 5) Notify Quick will load each file into our auto dialer system, and 
provide instructions to client for calling into the system and 
launching a dialing session.

 6) Premier lists will bypass all standard and express priority lists 
in the queue. 

 7) Additional lists. Clients may wish to create sub lists that are 
finer breaks within the overall list (for example, bus routes may each 
have their own list, or staff versus students, etc.) Each additional list 
is charged a one time $5 set up fee.

 8) File updates. Client may send updates as needed, at no additional 
cost, under the Premier plan.

 USAGE:

 1) When the client wishes to send announcements, they may call into
Notify Quick’s system from any touch tone phone. The client will be 
prompted to enter their client number, password, and list number on 
their phone keypad. 

 2) The client creates their own voice recording over the phone. The 
outgoing message should include the client’s name and phone number.

 3) The auto dialer will call each number on the list and play the client’s
pre-recorded message to live answers and answering machines. 

 4) A second pass through the list will attempt to reach the numbers 
that had a busy signal or no answer on the first pass.

 5) Clients may request a detailed call report after each session, 
at no charge. 

 6) No commercial solicitations are permitted. This service is intended 
to provide notification to client’s members who would reasonably 
welcome such calls.

 PRIVACY POLICY

 1) Phone numbers will not be shared or sold to third parties, and will 
only be used to place calls as directed by the client. No advertisements 
will be added to client’s messages.

 2) Notify Quick Corp maintains a Do Not Call list of call recipients who
have requested that we not call them again. These records are 
maintained for 10+ years, and calls will not be placed to phone numbers 
on this list.

 3) Clients are urged to advise call recipients of their ability to opt out 
from future calls. Recipients of live calls can press 9 to be added to 
our Do Not Call list, and recipients of machine messages can call us toll 
free at  1-877-441-6772 and request to be added to our Do Not Call 
list.  Customers who opt out will be screened out from future calls.
