
How To Process Your Nursing Application 1. Apply to appropriate schools or colleges, completing all testing and other requirements established by those institutions. 2. Complete and sign the scholarship application form. 3. Since this award is based on demonstrated financial need as well as scholastic excellence, you must submit a Free Application for Federal Student Aid (FAFSA) directly to the Foundation. If you have submitted a FAFSA to the Federal Student Aid Program for financial aid at individual colleges, just include a copy. This information should be based on the 2003 tax return filed by your parents and the one filed by you. You may be required to submit a copy of these tax returns to the Foundation for verification. 4. Give the completed application and activity sheet to your Guidance Counselor and ask your counselor to attach your high school transcript which must include your grades, rank in class, and SAT scores. Also include a recommendation from either your counselor or a teacher. 5. Your Guidance Counselor will forward your completed application to the Foundation as soon as all data is assembled. In order to meet the March 15th deadline, it is suggested that you submit all data to your counselor by March 1st. If you qualify as a candidate, you will be contacted for an interview. 6.
