
How To Process Your Medical School Application 1. Apply to medical schools, completing all testing and other requirements established by those institutions. 2. Complete and sign the scholarship application form. 3. Request that an Official Transcript be sent from both the college you are presently attending and one from your high school and have them sent directly to the Foundation no later than March 15. 4. Include a resume telling about your involvement in high school and college activities. Also comment on your decision to become a doctor/nurse. 5. Since this award is based on demonstrated financial need as well as scholastic excellence, you must submit the FAFSA directly to the Foundation completing all requested information including parents' income as well as student's income. This information should be based on the current (2004) tax returns filed by parents and the student. Be sure that all signatures are affixed to the form attesting to the information reported. You may be required to submit a copy of these tax returns to the Foundation for verification. 6. A completed application packet must be submitted to the Foundation no later than March 15, 2004 for consideration.
