Picking up right where Phantasm II left off, The Tall Man apparently returns with the help of the Spacegate. Mike, Reggie & Liz, who escaped from the flaming Perigord Mortuary, have been tricked by Alchemy. Reggie gets bruised, and the Hearse drives off carrying Mike & Liz. Reggie, while hurt, fortunately, is not dead. He runs off to where the Hearse crashed and exploded. Liz unfortunately is killed by a Dwarf, but for Mike, he's alive, but hurt. Reggie manages to rescue Mike from The Tall Man and his Minions. Mike is taken to a Hospital where he's treated for his injuries. He has a vision of the Other Side, and in that vision, sees his long-dead older brother, Jody. Mike awakens from the vision after The Tall Man apparently enters it and disrupts it. Mike is attacked by a Demon that disguised herself as a Hospital Nurse. Reggie makes it just in time and comes to Mike's aid.
The Demon Nurse, who is injured by Mike, bleeds to death and a silver Sphere emerges. This Sphere does not harm Mike, but it helps The Tall Man keep an eye on Mike. Reggie gets Mike out of the Hospital and takes him to Reggie's house. Over at Reggie's, there is a minor power outage that makes Reggie suspicious that The Tall Man is near. Mike finds Jody in Reggie's living room. Jody is back in the form of a Sphere, and when The Tall Man drops by, he tries to slow him down, but is roasted in the process. Reggie is knocked out, and Mike is taken from The Tall Man and enters an unknown and frightening Dimension. Reggie awakens only to find out Mike is missing, and that's because of The Tall Man. The "Jody-Sphere", while roasted, luckily for Reggie, isn't destroyed.
The Jody-Sphere tells Reggie to go to the town Holtsville. It doesn't tell Reggie why, but Reggie finds out for himself. He drives his new and restored HemiCuda to Holtsville, and in the process, meets up with Criminal Rufus and his partners in Crime Edna & Henry. They knock out Reggie and lock him
up in the trunk of his HemiCuda, and drive off. Rufus and Gang find an old house where they meet a young boy named Tim who kills Edna, injures Henry, and eventually does Rufus in, along with Henry, thanks to his trusty Magnum Pistol, Razor-Lined Frisbee, and Tomahawk. Tim frees Reggie,
and tells the sad and scary story of his family's demise, all caused by The Tall Man. The very next day, Reggie & Tim team-up (sort of) and drive across Holtsville. Reggie drops Tim off at a Holtsville Orphanage, but Tim stealthly sneaks into Reggie's 'Cuda. Reggie drives to and enters the Holtsville Mausoleum, armed with his Quadruple-Barreled Shotgun. There he is taken hostage by two african-american military women who tie him up. One of them is killed by a silver Sphere, the other bravely tries to fight it. Tim comes into the scene and blows the Sphere away with his Magnum. Reggie is un-cuffed. The remaining military woman, Rocky, pays her respects to her fallen comrade Tanesha while Tim & Reggie leave the Holtsville Mortuary and drive down the highway. The next day Rocky meets them again and they become a team. They head over to the town of Boulton, which is supposed to have a very large and gothic Mausoleum, and The Tall Man will more than likely be there. They enter Boulton and camp out. There Reggie has a "dream" that Jody takes him to a Dimension that is ruled by none other than The Tall Man. Mike is found there and is rescued. Reggie wakes up but finds out it wasn't really a dream, because Jody is there with a Sphere that is controlling the Spacegate
that leads to the very same Dimension Reggie was dreaming about. Mike escapes, and The Tall Man barely misses not stepping out the Gate. The group then battles little Hand Monsters and defeats them. They drive over to the Boulton Mausoleum, where they await The Tall Man. Mike learns the secret of the Spheres, but is captured by The Tall Man. Soon, Reggie, Rocky & Tim have to contend with a trio of Zombies and battle the ever vicious Spheres. Will they rescue Mike in time and possibly
destroy The Tall Man? Or will they become Zombie Drones of The Tall Man?
