"General endotracheal anesthesia was accomplished.  Ancef 1mg and Decadron 8mg were given intravenous. 1% Lidocaine with 1:100,000 epinephrine was infiltrated postauricular for a total of 6 cc.  

Percutaneous facial nerve monitoring leads were introduced to the corner of the right eye and mouth with grounding leads in the forehead and shoulder.  Continuous intraoperative facial nerve monitoring was performed for the next 2 hours."
