2. Look at the courses you did not perform so well in. List them
below with %.
3. What is the reason you didn't do well in the courses listed

4. What will you do to make changes? Be specific.
5. How will you implement the changes you specified in #4 above?
6. Have your parents help you set goals for the future....(summer
school and next year in school) and sign this paper showing that you involved
them in this process.

Always keep a copy of your work until you know you have received a mark
for it. Ask your teacher
if it is okay to delete your work files before you delete them. Teachers
don't fall for ......"I sent
it to you. Didn't you get it? I deleted the file because I handed it
in. Do I have to do it all over
again?"
