Department of Public Health, Chronic Disease Prevention Programs, 5 Randolph St., Canton, MA 02021. T: 781-774-6601. Hours of Operation Monday - Friday 8:30am to 5:00pm. I have a Masters in Clinical Social Work and my role is to work with all the regional coalitions/groups around chronic disease prevention programs by providing technical assistance and support to help sustain the groups that are active. The state has state plans for each program and within my role I attend all of the statewide meetings and bring back to the regional groups/coalitions this information as well as information about what is going on in the Southeast Regional communities as well as other regional communities in regards to chronic disease prevention programs. What is working/not working, what the communities are working on based on what they have identified as a problem and targeted group. The state DPH has many programs that it funds and works with and although funds have been cut tremendously in some areas, the state has many representatives that are actively working with communities. |
