| The Glide First Citizen Award is for:
* Outstanding activities serving the Glide area civic community * Service or contributions over multiple years and continuing to the present * A resident of the Glide area: Dixonville to Diamond Lake * Someone not previously honored with the Glide First Citizen Award Please tell the selection committee enough about the person’s activities and contributions that they can compare him or her to other nominees. We don’t want to limit your responses with a form. Use as much paper as you need. Please include all the information listed below. * Nominee’s Name * Nominee’s Address * Nominee’s occupation (previous, if retired) * Person submitting nomination: Name, phone and email (if available) * List organizations this nominee belongs to and other programs or projects this nominee has worked on. Examples of organizations: Glide Garden Club, Volunteer Fire Department. Examples of other programs or projects: Christmas Food Baskets, SMART, litter pickup. For each organization or program, please include as much information as you can: 1. Types of contributions: offices held, projects involved with, or services performed Examples: “Vice-president,” “led annual fund-raiser,” or “Works in kitchen at every function” 2. Dates and, if known, amount of time spent Examples: “2001-2004, 4 hours/week" or “Every Christmas season since 1999" 3. Unique talents, skills or effort provided Examples: “Trains new volunteers” or “Organizes the workers so everyone has a job” 4. Who has benefitted from this contribution, and how?
