If this letter is for admission to graduate school, please submit the appropriate forms I need to fill out on the same day that you send the recommendation letter via e-mail. Make sure your recommendation letter addresses the questions asked in the graduate application.
If the recommendation letter is going to be part of a placement file, then the form that you obtain from the placement center needs to be given to me.
In either case, you should type your information - don't print. You should waive your right to read the letter of recommendation, keeping in mind that I will still give you a copy for your records. In addition, the information pertaining to myself should be typed as well. The only thing I should need to do under my information is sign my name.
