Oesophageal carcinoma by vital staining

  Fifty-nine years old  patients was admitted because of chronic epigastric pain, loss of apetite and weight loss. He experienced his symptomatology for about four months. His clinical findings was normal. Erythrocyte sedimentation rate was 20mm/h. Haematologic studies, blood sugar, BUN, serum creatinine, uric acid,AST,ALT, alkaline phosphatase, serum proteins were all within normal limits. Abdominal ultrasound examination was normal.    

Upper gastrointestinal endoscopy revealed in the lower third of the oesophagus poorly demarcated nodular mucosal elevation. ( Panel A ).

After throught oesophageal mucosal wash with physiologic saline, mucosal vital dye staining with 1% Lugol solution was made. The mucosal lesion was well demarcated and unstained, while other parts of oesophageal mucosal lining were dark brown in coulor ( Panel B ). The lesion was 30mm in diameter Multiple mucosal forceps biopsies were performed. Histological report was: Carcinoma planocellulare non ceratodes in situ, focalis microinvasivum.

This is further to demonstrate the usefullness of vital dye staining of mucosal changes in precisely defining their extensity and relation with surrounding structures. This information are of high importance in performing exact topical biopsies and planning further radical oesophageal surgery.
