   Below is a checklist of things you must do and the dates by which you must do them.
   Please print this out and keep it handy.

   (an electronic copy of application must be sent to: [email protected])

   _____Passport numbers to: [email protected] (This is a deadline date to assure airline seats on
  group flights (if you don't have a passport already PLEASE APPLY IMMEDIATELY AND SEND APPLICATION 
  CONFIRMATION NUMBER AS SOON AS POSSIBLE)

   _____$385.00 deposit for optional Cuzco trip due

   _____ $1,000.00 due for Amazon program

   _____ $300.00 payment for optional Cuzco trip due
