When you get a club of a hammer, go back to the first level (Dan's Crypt) and near the gates where you started the game look for a fake wall you can destroy.
It will contain a life bottle.
(Probably the most overlooked life bottle in the game.)
If you get all the chalices you will get an extended ending.
