1. New owner agrees to take the puppy to her/his vet within 48 hours for a verification of health. We guarantee the puppy against any birth defects and/or genetic health problem which the vet may discover during the first year. This puppy has been seen by our vet and has received the scheduled puppy shots. The dew claws were___ were not___ removed three days after birth.

2. New owner has been advised not to let the new puppy mix with other animals, dogs/puppies, until puppy is 4-5 months old and has had all shots. Also puppy is not to go to a groomer or kennel until he/she has received all shots including rabies shot.

3. We feed puppy dry kibble --Eukanuba small bites for the first year. We do not feed them people food. Doggie treats are good occasionally. FAT IS NOT GOOD. CHOCOLATE IS POISON TO LITTLE DOGS.

4, If you are trying to train your puppy to relieve himself in a special place, we recommend you take him to that place upon waking, after breakfast, and anytime he sleeps or eats. He will eat every three to four hours and at this age take a morning and afternoon nap. Be sure to tell him how good he is when he uses the newspaper. The newspaper and food are kept at opposite ends of the area. We do not give the puppy freedom to wander about until he is trained and stops chewing on electric cords. We leave water down at all times until an hour or two before bed time. We only leave food down for 30 minutes and then remove it to control the potty training. A metal (or plastic) playpen will confine him to an area like the kitchen until he is safe in the public rooms. You should supervise him as you would a two year old child. This puppy has______ has not _____ been crate trained.

5. If your puppy is to use a crate, remember his crate is a sanctuary that you may not drag him from. If he gets inside he is safe (much like tag.) Never hit your baby. Your voice is all the praise or reprimand he will ever need. He loves you.

When you receive your puppy you will also receive the pet health record from______________________. You will also receive the AKC REGISTRATION APPLICATION which I will have filled out the back. You must fill out the application and send it in within one year to receive you pet's pedigree. Upon request, you can go back four generations.

I agree to pay a deposit of $200.00 to reserve my puppy which will be applied to the total. Then I will pay the remaining balance at time of pick-up or before my puppy is Shipped to me. I have read the above agreement and accept the conditions of this contract.
