| As a part of the Philadelphia School District's policy, all volunteers are asked to complete the Act 34 (Pennsylvania Child Abuse History Check) and Act 151 (Request for Criminal Record Check) forms that are required of all SDP employees. This notice is just a reminder that these forms must be completed in order to serve as a volunteer or classroom visitor (i.e. chaperone, classroom volunteer, guest reader, special event volunteer, H&S Board member, etc) within the school district. If you have not already taken care of this requirement, we ask you to do so immediately. Clearance application forms are available in the school office. However, in order to make this process even more convenient for you, you may click on the link at the bottom of this page to print out copies of the forms. Please note there is a $10 fee for each clearance and it must be a certified check or money order. Please contact the Main office at school if you have any questions.
