With the introduction of the Barret Model 121, Heckler & Koch have designed a better .50 cal sniper rifle based on the old M90 of the 20th century. Using a ceramic frame to reduce weight, along with other measures, the MSG65 is the lightest of sniper rifles (the "Heavy" referring to the caliber rather than actual weight) in it's class. It also uses a bullpup frame to decrease (by not much) the weapon's size. Other accessories include a free-floating barrel, that is fluted to offer strength as well as keeping the barrel temperature low. It uses a delayed blowback operation, and the barrel ends in an Improved Gas-Vent III system, effectively reducing any muzzle jump. A bipod is fitted on the under-barrel fork for additional stability when laying prone. The scope handle can accept any type of scope, but mounts an Image Magnification-3 as standard. The top of the mount can accept the GN45 scope system.

The free-floating barrel offers an additional die when firing. Unfolding the bipod offers 2 points, and the weapon has a recoil of 2 points per round.
