In all reports please include the following information.
1)your first and last name
2)Area fished (for saltwater reports I PREFUR area number + name but just name will do) Just a name is fine for rivers and streams.
3)Date(s) you fished
4)your report.

Ex.
Jon Doe
area 8-2 Humpy Hollow
date:0/0/00
Went out caught some fish.... etc. etc.
