ARTICLE 15 REPLY DOCUMENTS
WRITTEN STATEMENT TO THE COMMANDER
This is to be used as a guide. What you want to do is to present your case on paper to your commander as clearly and persuasively as possible,; about four to five paragraphs should be enough. If co-workers and supervisors will write statements for you attach them to your statement. Their statements should hit your outstanding achievements on and off duty.
1. Background. This statement concerns punishment under Article 15 offered to me by the commander of (squadron) on (date) for the offense of (fill in from AF Form 3070) on (date). I asked the commander to consider the following information.
2. The Facts:
....a. On (date), at (location), I was doing (what).
....b. If anyone else was involved explain how.
....c. Explain why you acted as you did in this instance.
....d. Other factors you want the commander to know.
3. Statement of Not Guilty: If you desire to plead not guilty, explain why, for example: I was not present when the accident happened; I did not speak to the SP as they say I did; I was present for duty but got called before roll call; I had authority to act as I did from __________; any other explanation.
4. Statement of Guilty: If you desire to plead guilty, do so simply, for example: I know I did wrong and I�m sorry; I have made a big mistake and want to put it behind me; I have learned my lesson from this experience; Nothing like this will happen again.
5. Duty performance:

....b. Special honors received.
....c. Letters of appreciation or commendation.
....d. Performance Report ratings.
....e. Comments from co-workers and supervisors.
....f. Don�t be afraid to blow your own horn here.
6. Appropriate punishments.

....b. Use attached financial statement to show how forfeiture would affect you?
....c. How will correctional custody affect you, wife, kids, pets?
