| In order to confirm your status as an active volunteer you must: 1) View the Volunteer Responsibilities Webpage. 2) Either fill out the Volunteer Information Form on the Internet OR manually through a card handed out by one of the house coordinators. 3) Print out, Sign the Authorization for Minor Child Participation and/or the Consent for Medical Care on the Internet and return to one of the house coordinators OR manually through a form handed out by one of the house coordinators. Once you have completed and turned in all of the above, you can start! |
