| FACILITY PROGRAM DESCRIPTION The Emerald Retirement Residence is a facility established to administer and maintain a fully licensed facility for the purpose of administering and providing social, physical, support to adults, 60 years or older, ambulatory and non-ambulatory, who depend on assistance in their daily living activities. Included in that purpose is the respect and acknowledgement of wishes, desires and need of each client in order to retain their sense of personal dignity. Emerald Retirement Residence encourages Senior's active participation with options for enhanced living in areas of nutritious meals, entertainment, education and safety and other creative endeavors one desires to seek. BASIC PRINCIPLE a) The basic principle for the operation of the facility shall be to provide the kind of personal care normally provided by a close relative to an aged family member. b) Emerald Retirement Residence shall provide the Basic Services ( cf.sec 87101.7) Care and Supervision ( cf.sec 97101.9) and planned Activities ( cf.sec 87579) for the ambulatory and non-ambulatory persons, 60 years or over ( cf.sec 87101.32 and cf.sec 87101.18) . c) The facility shall be opened 24 hours a day and 7 days a week. GOALS 1. To established and maintain a fully licensed Residential Care Facility for the State of California. 2. To provide quality service to the client's satisfaction and their families. 3. To maintain a positive relationship with each client and their families and to anticipate and respond to their needs and desires within our scope of services. 4. To encourage family members to visit regularly ( visiting hours are from 9:00AM to 9:00PM) , to participate and get involved with client's activities in or outside the facility. For special occasions such as birthdays, holidays, barbeque or picnic, client's families shall be invited to join. 5. To provide the following services: BASIC SERVICES; 1. Will provide a safe and healthy living conditions for six adult, sixty years (60) and above. 2. Will provide three (3) nutritionally balanced meals, plus morning, afternoon and evening snacks. In addition, there will be provision for modified diets prescribed as a medical necessity. 3. Personal assistance and care as needed by the client, such as dressing, grooming, eating bathing and assistance with taking prescribed medication. 4. Laundry and bed linen services shall be provided at least once a week or as often as needed. 5. Plan, arrange and or provide transportation to medical and dental appointments, and activities outside the facility. 6. Will provide planned activities including utilization of the community resources such as library, senior centers, parks, recreation centers, theaters, Red Cross and other community places. Planned activities shall include social, educational, recreational activities appropriate to the interest and capabilities of each client. 7. Notification to family members and other appropriate persons/ agency of the client's needs. 8. Bedside care for minor temporary illness. 9. Maintenance and supervision of client's cash or property if necessary. 10. Client's room shall be cleaned at least once a week or as often as needed. EDUCATIONAL ACTIVITIES SHALL INCLUDE THE FF: " Classes conducted by the Self Help for the Elderly, Senior Focus, American Red Cross, Recreation, Senior Centers and Hospitals. " Each client will be encouraged to attend and or visit places of interest by arrangement. These places of interest shall include but not limited to the theater, concerts, plays, opera, museum, park, race track, bay cruises, senior center or church bingo, religious services, senior meetings and forum. San Mateo Senior Center, Alameda de las Pulgas, King Community Center, Senior Focus, Foster City Recreation Center offer variety of activities on a daily basis which are designed to enhance the life of each participant. Emerald Retirement Residence's plan is to interface with these community resources on a regular basis so that clients will increase their social awareness and discover activities that will suit their interests. The administrator will provide for the transportation and will accompany the clients on all activities outside the facility. Newspaper, magazines and journals will also be provided in the facility. Books will be made available from the public library upon request. Cable TV, VCR, Stereo will also be made available in the facility. Current movies that are available in the video stores will also be made available upon request. Each client will also have the option to bring their own private phones at their own expense. |
| OPTIONAL SERVICES Cosmetology Services such as hair cut, styling, perm, tinting/coloring, nails and other related services will be provided at the client's expense. Appointments shall be made as needed by clients. Fees are to be paid directly to the service provider upon delivery of the service. State license cosmetologist will be used. |
| TRANSPORTATION POLICY 1. The facility will provide the Clients with transportation for medical, dental appointments and planned activities outside the home. Upon request, all information regarding the city transit system will be provided to clients. The facility will also provide transportation to the clients for their social, recreational and religious activities. 2. All employees who transport clients must have a current driver's license, proof of insurance, a good driving record and is expected to drive clients safely. There must be copies of driver's license and proof of insurance in the employee's file. 3. The vehicle used in transporting clients will be maintained in good condition and must be routinely checked. 4. Seatbelts must be used by all occupants of the vehicle. |
| HOUSE RULES AND REGULATIONS I. Visiting hours are from 9:00am to 9:00pm daily. Other hours can be arranged upon request. II. No smoking is allowed inside the facility. III. Clients are not allowed to enter other Client's room without the client's consent. IV. Alcoholic beverages and illegal drugs are not allowed in the facility. V. Quiet time (i.e. no loud conversations and laughter, loud TV or radio) must be observed after 10:00pm. VI. Physical, mental, verbal or any kind of abuse to anybody is not allowed. VII. Personal TV or radio should be used to a minimum so as not to disturb other clients. VIII. Clients must sign out the log book when leaving the facility and must sign upon arrival. IX. No visitors will be allowed in the client's room overnight. |
