| ADMISSION POLICIES CRITERIA FOR INDIVIDUALS ACCEPTED: All residents are admitted without regard to race, color, religion, ancestry or national origin. -Persons capable of administering their own medication. -Persons receiving medical treatment and care outside the facility or who are receiving needed medical care from a visiting nurse. -Persons who because of forgetfulness or physical limitations need only be reminded or to be assisted to take medication usually prescribed for self-administration. -Persons with mild problems such as forgetfulness, wandering, confusion, irritability, and inability to manage money. -Persons with mild temporary emotional disturbance resulting from personal loss or change in living arrangement. -Persons with mild ambulatory or non-ambulatory and persons who are under 60 years of age whose needs are compatible with other residents in care, if they require the same amount of care and supervision as do other residents in the facility PERSONS WHO ARE NOT ACCEPTED OR RETAINED ARE THE FF. : -Persons who has active communicable disease. -Persons who require 24 hours, skilled nursing or immediate care -Person who is not an elderly and has needs which are in conflict with other residents or the program or a service offered, or requires more care and supervision than other residents. PRE-ADMISSION APPRAISAL Prior to admission the prospective client and his/ her responsible person, if any shall be interviewed by the administrator to determine if the facility situation matches the needs of the client using form LIC-603. Sufficient information about the facility and its services shall be provided to enable all persons involved in the placement to make an informed decision regarding admission. The prospective client's desires regarding admission, and his/ her background Including any specific service needs, medical background and functional Limitation shall be discussed. A discussion with previous caregiver and / or family member regarding specifics about the client's lifestyle, likes/ dislikes on food, bathing habits ( eg. Time of the day client takes his/ her bath) limitations on daily activities, physical, psychological and social particulars. Prior to admission a determination of the client's suitability for admission shall be completed by a licensed physician using form LIC-602. Client or responsible person will be given the opportunity to visit the facility prior to admission, to personally acquaint the client with the facility's setting and meet other clients. No person shall be admitted without his/ her consent and agreement or that of his/ her responsible person if any. PHYSICAL EXAMINATION (including TB testing) A. Prior to a person's acceptance as a resident, the Licensee, shall obtain and keep on file, documentation of a medical assessment, signed by a physician within the last year. B. The medical assessment, signed by a physician within the last year. a) A physical examination of the client indicating the physician's primary and secondary diagnosis, if any and results of a examination for communicable or other medical conditions which would preclude care of the person by the facility. b) Documentation of prior medical services, history and current medical status including but not limited to height, weight and blood pressure. c) A record of current prescribed medications and an indication of whether the medication should be centrally stored. d) Identification of the person's physical limitations to determine his/ her capability to participate in the program provided by the facility, including any medical diet limitations. e) A determination of the person's ambulatory status. f) Any other information applicable to the preadmission appraisal. ADMISSION PROCESS 1. Prior to accepting a resident for care and in order to evaluate his/ her suitability, the Administrator shall do the following: a) Conduct an interview with the applicant and his responsible person or agency. b) Perform a pre-admission appraisal using LIC 603. c) Obtain and evaluate a recent medical assessment using a physician's report LIC 602 and must specifically state that client is negative to any communicable disease like tuberculosis. d) Identification and Emergency Information LIC 601 shall be completed and maintained in the facility and will be readily be made available to the person in-charge but accessible to unauthorized person. All information must be current. e) Personal rights LIC 613 will be reviewed, completed and signed by each client and responsible person and a copy will be given to the client, responsible person and the facility. Clients will be assured that their rights will not be violated. f) Clients and responsible person shall read and sign the house rules. The house rules and personal rights will be posted on the facility's bulletin board. g) Client's personal property and valuables shall be completed on admission LIC 621. Any addition or reduction will be noted and initialed by the client or responsible person and administrator. h) The admission agreement shall be reviewed and executed by all parties. THE ADMISSION POLICIES SHALL BE REVIEWED AND SIGNED BY ALL PARTIES AND A COPY WILL BE KEPT IN THE CLIENT'S FILE.
