By signing this Ultimate Elvis Tribute Artist Contest Release Form, I agree to the following:
1. I agree to be bound by all Ultimate Elvis Tribute Artist Contest Official Rules and Guidelines.
2. I agree that my performance shall not in any way be derogatory to Elvis Presley, his heirs, the contest
organizer, or Elvis Presley Enterprises, Inc. or any of its affiliates.
3. I grant Elvis Presley Enterprises, Inc. the right to use my name, image, likeness, biography, and any
photographs or audiovisual footage of my participation in the preliminary round of the Ultimate Elvis
Tribute Artist Contest and the semi-finals and finals (if applicable) of the Ultimate Elvis Tribute Artist
Contest, including my performance, presence and/or interview without the payment of any
compensation, in connection with the advertising and promotion of the Ultimate Elvis Tribute Artist
Contest, including without limitation, any preliminary rounds of the contest as well as the semi-finals
and finals of the contest to be held in Memphis, Tennessee in August 2009.
4. I grant Elvis Presley Enterprises, Inc., the irrevocable right to record my participation in the preliminary
round of the Ultimate Elvis Tribute Artist Contest and the semi-finals and finals (if applicable) of the
Ultimate Elvis Tribute Artist Contest, including my performance, presence and/or interview, and use,
adapt, reproduce, distribute, broadcast and exhibit my performance in perpetuity in any and all media,
manners and platforms, without the payment of any compensation.
5. I acknowledge that before I participate in the Ultimate Elvis Tribute Artist semi-finals in Memphis,
Tennessee, I will sign an Ultimate Elvis Tribute Artist Participant Agreement and Release, which will be
provided to me by EPE.
6. By signing below, I am expressly releasing he contest organizer, Elvis
Presley Enterprises, Inc., their respective parents, affiliates, officers, directors, employees, and agents,
from any and all claims which I may have for invasion of privacy, right of publicity, defamation,
violation of my moral rights, copyright infringement, or any other causes of action arising out of the use,
adaptation, reproduction, distribution, broadcast or exhibition of any photographs or audiovisual footage
of my performance.
