Interlocking nails may be used in very distal tibial fractures provided that the distal sement can hold the 2 locking screws
This patient had in addition a subtrochanteric fracture which was fixed by a Gamma nail
Due to severe osteoporosis, a shatter occured in the proximal segment during insertion of the nail..
However, this did not compromise the fixation
