To whom it may concern;
This is to inform you that in my opinion, (insert name of school corporation) is not complying with the Indiana's requirements for a free, appropriate, public education for (insert name of student or students), as stated in Article 7.
I am filing a complaint because I believe the following practices or actions are in violation of Article 7:

You may fax your complaint but we only suggest that if you have very little information to send.

Once they receive your complaint a Complaint Investigator will contact you within 1 to 2 days, depending on weekends and holidays this time may be longer.
