TO ALL WHOM IT MAY CONCERN
Know Ye, That JOHN A. MAGGARD, a Private of Captain Shadrach Combs Company (D) Three Forks Battalion of Kentucky State Troops Volunteers, who was enrolled on the 7th day of October, one thousand eight hundred and sixty four, to serve six months, or during the war, is hereby DISCHARGED from the service of the State Of Kentucky this 17th day of July 1865, at Irvine, Kentucky, by reason of expiration term of service.
Said John A. Maggard was born in Perry County in the State of Kentucky, is 32 years of age, 5 feet 10 inches high, dark complexion, black eyes, dark hair, and by occupation, when enrolled, a farmer

DECLARATION FOR PENSION

On this 11th day of April, A.D. one thousand nine hundred and Seven, personally appeared before me, a County Court Clerk within and for the County and State aforesaid, JOHN A. MAGGARD, who, being duly sworn according to law, declares that he is 74 years of age, and a resident of Hyden, County of Leslie, State of Kentucky; and that he is the identical person who was ENROLLED at the mouth of Wootons Creek, Perry County, Kentucky under the name of JOHN A. MAGGARD, on the 7th day of October, 1864, as a private, in Company D, Three Forks Battalion Volunteers in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Camp Irvine, Kentucky, on the 17th day of July, 1865.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 10 inches; Complexion, dark; Color of Eyes, Black; Color of Hair, dark; that his occupation was Farmer; that he was born 28th day of September 1832, at Perry County Kentucky.
That his several places of residence since leaving the service have been as follows: Perry and Leslie Counties Kentucky.
That he is not a pensioner.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of February 6, 1907.
That his post-office address is Hyden, County of Leslie, State of Kentucky.

Maggard, the claimant, (make his mark) to the foregoing declaration; that they have every reason to believe, from the appearance of the claimant and their acquaintance with him of 20 years and 25 years, respectively, that he is the identical person he represents himself to be, and they have no interest in the prosecution of this claim.

On this the tenth day of July 1874 personally appeared before me Clerk of the Leslie County Court, a Court of record in and for the County and State aforesaid, JOHN A. MAGGARD, who, being by me, duly sworn, states that he has never been in the military or naval service since his discharge from the army to wit0 July 17th, 1865, nor ever was treated in any hospital for the disease for which he now claims pension.

DECLARATION FOR INVALID PENSION

On this 26th day of July, A.D. one thousand eight hundred and Seventy Nine personally appeared before me, Clerk of the Leslie County Court, a court of record within and for the County and State aforesaid, JOHN A. MAGGARD, aged 46 years, a resident of the County of Leslie in the State of Kentucky, who, being duly sworn according to law, declares that he is the identical John A. Maggard who enlisted in the service of the United States at Perry County in the State of Kentucky on or about the 7th day of October in the year 1864, as a Private in Company d commanded by Captain Shadrach Combs in the Three Regiment of the Forks Battalion Kentucky State Troops commanded by E.B. Greenway in the war of 1861, and was honorably discharged at Irvine in the State of Kentucky on or about the 17th day of July in the year one thousand eight hundred and sixty five, that while in the service aforesaid, and in the line of duty that while in line of duty about the 1st of June 1865 at Irvine Kentucky he was taken sick with sore throat and tongue and breast the disease was called black tongue fever by the regimental physician. Said disease was enjoined from exposure of camp life such as he was not accustomed to and it affects him at this time in the breast, throat and nose
That since leaving the said service, this applicant has resided in the Counties of Perry and Leslie in the State of Kentucky and his occupation has been that of a farmer of a very light character
That prior to his entry into the service above named, he was a man of good, sound physical health, being, when enrolled, a farmer
That now he is three fourths disabled from obtaining his subsistence from manual labor in consequence of his above named injuries received in the service of the United States.
He makes this declaration for the purpose of being placed on the Invalid Pension Roll of the United States, by reason of the disabilities above stated, and revoking and countermanding all other authority that may have been given, he hereby constitutes and appoints with full power of substitution and revocation P H Cooney of Washington, D.C. his true and lawful attorney, to prosecute his claim and obtain the Pension Certificate that may be issued. That his Post office is at Manchester, County of Clay, State of Kentucky
That his domicile or place of adobe is on Cutshin Creek, Leslie County, Ky.

My post office address is Hyden, Leslie County, Kentucky
My occupation is that of farmer, I was born in Perry County on the 28 day of September 1832. My personal description is as follows: Height 5 feet 10 inches; complexion dark; color of hair dark and of eyes black.
I am married; my wifes name is Matilda Maggard
My places of residence since the war have been; from about 1865 to about 1898, in Perry and Leslie County, State of Kentucky, my P.O. address being Hazard from about 1865 to 1878, in Perry County, State of Kentucky, my P.O. address being Hazard; from about 1878 to about 1890 in Leslie County, State of Kentucky, my P.O. address being Hyden, Kentucky
The following statement of all the service ever rendered by me in the military or naval service of the United States: I enlisted at Wootons Creek, on the 7th day of October 1864, to serve 6 moths, in Company D of the Three Forks Battalion of Kentucky Volunteers and was honorably discharged there from at Irvine, Kentucky on the 17th day of July 1865
I now have the following injuries, diseases, or disabilities, each of which was caused or incurred as stated opposite thereto and neither of which is the result of vicious habits on my part

Maggard, the claimant, sign his name to the foregoing declaration; that they have every reason to believe from the appearance of said claimant and their acquaintance with him that he is the identical person he represents himself to be; that they have resided in the same neighborhood with said ex-soldier for the past twenty years, and they are acquainted with his habits and his capacity for work, and they say that, by reason of the disabilities for which he claims pension in the foregoing statement, he is incapacitated for manual labor to such a degree as to render him unable to earn a support, and that none of said disabilities is the result of vicious habits on his part; and that they have no interest in the prosecution of his claim.

POWER OF ATTORNEY
Know all men by these presents, That I JOHN A. MAGGARD, a Private soldier late Company D regiment Three Forks Battalion Volunteers of Kentucky in the County of Leslie and State of Kentucky have made, constituted, and appointed, and by these presents do make, constitute and appoint W.S. Odell & Co. of Washington, D.C. my true and lawful attorneys, for me and in my name, place, and stead, hereby annulling and revoking all former Powers of Attorney or authorizations whatever in the premises, to prosecute before any Department, or the Courts, or Committees of Congress of the United States until final completion, for me, my claim for Breast complaint and affected head and to, from time to time, furnish and further evidence necessary or that may be demanded, giving and granting to my said attorneys full power and authority to present and verify any petition or replication and to do and perform all and every act and thing whatsoever requisite or necessary to be done in and about the premises, as fully to all intents and purposes as I might or could do if personally present at the doing thereof, with full power of substitution and revocation, hereby ratifying and confirming all that my said attorneys or their substitute, may or shall lawfully do or cause to be done by virtue hereof, and the said attorneys, or if they should die their legal representative is authorized to receive any draft or warrant that may be issued in settlement of said claim; also in consideration of the obligations assumed on the part of said attorneys, this Power of Attorney is irrevocable by me, and any person who may have rights under the undersigned, shall recognize the same until said claim is settled and the draft issued in payment thereof is received by said attorneys, or their legal representative or substitute.
