November 15, 2003 A few important reminders: Rep Fee and Team Fee - If you have not already done so, here is a reminder to please pay the rep fee of $100 and at least $50 towards the t$150 team fee this weekend. This is our final deadline. You can give the oney to me at the game in Berwick on Sunday. Health Form - If you have not already done so, please pass in the health form with all the relevant info re your child. Alan and I will have those forms at all games. In the event of any accident that requires m3edical follow up from the rink, we will provide the form if needed. If you need another copy of the form, please see me. Xmas Basket - please do not forget to hand in your contribution to the Xmas basket to either Gail Wittenburg or Alana ASAP. Website Information - Mary Lou Conrad is going to post info re each player on the website, and she made up some special forms that need to be filled out. If you have not already done so, pleas pass it in! If you need another form, I will hopefully have some more at the game in Berwick. Or you can ask MaryLou directly.
