We will be using addresses and phone numbers throughout the year for different purposes (addressing envelopes, creating a phone book, etc.). If you have any concerns about your address/phone number being used, please let me know.
My child may be photographed and photos used for school purposes (classroom newsletter, class website, newspaper, etc.)
I am available for helping in the classroom, field trips, parties, etc. (I'll contact you regarding days/times available.)
I need my newsletters sent home on paper (hard copy). (Otherwise, newsletters are sent home via e-mail (pdf) and posted on our class website.)
I am interested in getting involved with our school's PTO.
