of acute and chronic illnesses.

activities for a
wide variety of health issues such as
high blood pressure, diabetes, heart problems, and cancer.
= Family planning, birth control, and routine
GYN
services.
= Pediatrics, immunizations, and complete
health care
for children.
= Assistance with accessing community-based
health and
social services.
In addition, Tri-State offers services not usually
available in
other doctor's offices including
assistance with application for CHIP, Medicaid,
and other benefits, and a discount program
based on income level and family size.
Your Rights and
Responsibilities:
We are glad that you have
chosen to
receive your health care at Tri-State. Because we
value your business we have developed the following guidelines to help
avoid misunder-
standings and to ensure that we meet your health care needs to the best
of our
ability.
1. Speak up if you have any
questions or
concerns.
Choose a health care provider who you feel
comfortable talking
to about your health and
treatment. At Tri-State, we have many providers from which to
choose. It's okay to ask
questions and expect answers you can understand from your health care
team.
If for any reason you are not happy with your experience
at
Tri-State, you can:
--- ask any
staff
member to give you a patient feedback form which are
reviewed daily by the management staff;
--- fill out
a
patient comment card which is available on the checkout desk;
--- or ask to
speak
with the Practice Manager.
We appreciate your input --- it is the only way we can
improve
Tri-State for everyone.
2. Keep a list of the medications
you take.
This is especially helpful when you go to other
facilities,
such as the hospital, for additional
care. Keep this list with you at all times and share it with any
doctor
who is involved in
your treatment.
3. Make sure you get the results of
any test
or procedure.
As a general rule, we attempt to notify you about
tests within
2 working days after we
receive the results. However, please keep in mind that when a
test is
performed at the
hospital or another facility, we have to wait until they notify us of
the
results before we
can contact you.
4. Talk with your health care team
about
your options.
It is important that you understand your choices.
5. Calling on evenings and weekends.
A liscensed health care
provider is
available to speak with you 24 hours a day through our
on-call service. However, please limit weekend or evening calls
to
serious medical issues
that can not wait until normal office hours. The on-call provider
can not
authorize refills
of prescriptions, give out test results, or
order labs
or medications. This must be done
during regular office hours.

Tri-State Discount Programs
Did you know that Tri-State
offers discounts
to individuals and families based on family
size and income
level? This is made possible through a federal grant intended to
help
reduce health
care costs
for people with no health insurance or limited incomes. As a
commumity
based non-profit organization, Tri-State is committed to offering
health care
to all area
residents
regardless of their ability to pay.
To see if you qualify for our sliding scale discount program, please
visit or call the
Tri-State Case
Manager
at ( 301 ) 729-6730. She is available to meet with you and
discuss the various options for meeting your needs. In addition,
the Case
Manager can
assist you in taking advantage of many other valuable health and social
programs such as
pharmaceutical assistance programs for those on limited incomes needing
prescription
medications, the Child Health Insurance Program, Right from the Start,
WIC,
supplemental
food and heating assistance, mental health and specialty care,
and other
health programs
such as smoking and substance abuse counseling.
If you are uninsured but don't want to apply for our
discount
program or are not eligible,
you can automatically receive a 20% discount if you pay your bill in
full at
the time of your
visit. This is our way of helping to ensure that all
uninsured residents have access to afford-
able health care.
