| Client Family Name: Doyle Child(ren) at time of care: 1 Female 3 years old Area of Care: Oshawa Duration of Care: 1 month in 2005 Details: Parents are separated. Mother does not want child in care. Father and Grandmother do want child in care. Grandmother came for interview. Father enrolled child. Mother agreed to try care but warned that she has always pulled child out of care within 1 month. Sometimes as quickly as 2 days. Mother reported that they had tried childcare centers before and were not happy with them. Both mother and father agreed to policies and procedures as outlined in handbook and provider had a signed contract with both parents. Then provider discovered that it was the Grandmother paying for the childcare. Grandmother did not like the idea of paying for days child was not there. Care Terminated by: Provider at end of 4 week trial period Reason for termination: Situation noted above and payment failure |

| Client Family Name: Reid Child(ren) at time of care: 1 male 2.5 years old, 1 male 1 year old Area of Care: Whitby Duration of Care: 4.5 months on average per provider Details: Parents did not submitt a deposit and questioned all the providers policies regarding MONEY. After care was terminated the family found a new provider and gave the new provider grief around money issues as well. Parents have gone through 4 providers since starting oldest child in daycare at 1 year old. 4 providers in just 1.5 years and the numbers are still climbing. Past and current providers found out that mother was spreading lies about them. Care Terminated by: Parents & Providers Reason for termination: Parents disregards for policies and fees |
| Client Family Name:Pawis/Casey Child(ren) at time of care: 1 male 5 years old Area of Care: Oshawa Duration of Care: 1 month & 6 months Details: Family started care with provider in May. Mother refused to pay provider for sick days even after signing providers contract agreeing to do so. Provider had communication issues from the beginning with family. Provider gave mother a notice and refused care until proper payment was received. Provider received payment then mother terminate care in June. Mother came back on Labour Day weekend while childcare was closed to ask provider if space was available for the child again to start on the following Tuesday. Space was available and mother signed all providers paperwork once again. Provider continued to encounter problems with mother neglecting the details of the contract. Mother wanted provider to lie to child's father about childcare rates so that he would have to pay the majority of the fees (as per court order father pays half of childcare fees). Mother called another provider to inquire about services. Mother then reported untrue things about old provider to the new provider. Both providers knew each other and are friends so child and situation was known by new provider. Family was not accepted into care by new provider. Care Terminated by: Parent Reason for termination: Mother terminated care for being charged a $5.00 late fee (as per contract). |
