PATHOLOG OF PREGNANCY.

1. INTRODUCTION:
Before I start with the real disease, complications and labour (delivery). I would like to explaine other changes during pregnancy which are important to know.
1. The total metabolism (carbohydrat, protein and fat) is increased in pregnancy due largely to your baby example: oxygen consumption is raised by 20% .
2. Thyroid gland enlarged in 70%.
3. Respiratory changes.
Movement of diaphragm increased, this will help gaseous interchange
so your baby will receive normal quantity of oxygen and normal excretion of carbon dioxide.
3. Cardiovascular system.
Blood volume, plasma are increased and this will lead into increased cardiac output, and other changes in the blood values especialy haemoglobin and you may become anaemic.
4. Gastro-intestinal.
It is minor degree mainly in the decreased motility due to increased circulating hormones steroid, and the same changes in the intestins tend to induce constipation and also some nausea and vomiting especialy in early pregnancy and in the morning.
5. Renal changes.
Frequency of micturition, urine infection and renal colic due to the compression of pregnancy  on the kidney and pelvis.
6. Sleeping.
You must have enough sleep, relaxation and raising your legs every day after noon at least 2 houres so the exchange of gas, oxygen, CO2 and nutrition, protein, carbuhydrat, etc... will be permitted easily to your baby and by sleeping enough this will reduce the tiredness and you will have very good dreams.

7. Travelling.
It is normaly permitted for normal pregnancy to travel by car but not more than 80 km.
also travel by airplane is permitted until 26 weeks of pregnancy and you should have a medical certificat for this and must inform the person concern. There are some disadvanteges in air travel:
1. changes in air pressure of course pressure in the plane cabin is adjusted but this is not always adequate for the baby .
2. seat it is preferably large comfortable and if possible in the first class or in the first line after the first classe so you can strech and moving your legs easily to prevent any pooling blood in the circulation especialy in the legs to avoid any problem of thromboses.
3. seat belt must be low as much as possible near the thighs.

I am going to reassure you that pregnancy is a physiological processe and I am going to explaine the most important ones which you may face them and I will put them for you in a very very simple way so you will understand them very well. Let us start with.

2.  PSYCHOLOGY:

As you know, and as I stated for you, that pregnancy is a physiological phenomen (and you know  perhaps you will have a boy and this boy may will be the presidant of your country) therefor you must expect some biochemical and psychological changes during this pregnancy. recent research has shown that our brain is the most sophisticated computer and has the ability to calculate and to register any behaviour you do through all the period of your pregnancy withot telling you, therefor you must always thing of good things and always very optimistic and look at the bottle that always half of it full and not empty, on this base the result of your computer will be positive and you will have noemal pregnancy without any complication.

3. ANAEMIA:

As I mentioned above that the mother demands more nutritions (IRON, FOLIC ACID, VIT.B GROUP AND PROTEIN) and their deficiency will lead into ANAEMIA. Therefor test of Haemoglobin and iron is very important through all the pregnancy. Usually you do not have  much symptoms but if anaemia is severe you will look pale, tired short breath and nervouse and of course is not good for your baby because of lack of oxygen to reach him.
You must have nery good and balanced food also you should take every day one tablet of iron and one tab. of folic acid regularely with one cup of orange juse because iron needs vit.c  to be absorbed and to fix it in the blood.

4.    VAGINAL DISCHARGES:

THE VAGINA, is naturally protected  by its normal flora, its special structure and by its PH which is between 4.2-4.7 it means acid and this a strong hold against any foreign attack, however, I would like to emphasize to take things easy by knowing the following:

1. NORMAL DISCHARGE.
Is white, made up of coagulated cervical secretion and epithilial cells. It is normally increased before menstruation and during pregnancy (And when it is increased we called LEUCORRHOEA(LUKORIA)).

2. YELLOW DISCHARGE.
Purulent discharge suggests cervical and vaginal infection (it can be caused by simple non- specific infection such as staphilococci, streptococci, etc...).

3. GREENISH DISCHARG.
Irritant vacuolated discharge suggest trichomonal infection. It is caused by small parasite called tricomonas consist of cytoplase, nucleuos and flagilles is not dangerous but must be treated well because can infect the husband as well and both partner must be treated

4. WHITE CURDY DISCHARGE.

Irritant discharge suggest MONILIA infection (Caused by candida albicans). Which is very common YEAST infection. Also must be treated

5. BLOOD STAINED DISCHARGE.

Please do not panic it is common in many women, because it is most likely to be due to cervical or vaginal infection.

5.   VAGINAL BLEEDING:

This subject is very important for every mother (if you you have even very little bleeding during your pregnancy you can contact me immediately on doctor4). The first thing you must do you should lie down on your back and do not panic and then

Bleeding is important and can be treated easily and successfuly without any complication if you take it easy without panic, therefor, if you read the following and understand them well you will be asolutely confidant and more relax:

1. BLEEDING AFTER INTERCOURSE.

This is not seriouse because it can happen from simple trauma, simple vaginal and cervical infection or simply from irritation during coitus.

2. BLEEDING FROM ECTOPIC PREGNANCY.

This bleeding by any mean is the most important, but it is easy to diagnose and its treatment is very effecace. It is usually in early pregnancy from the day of fertilisation until the first 4 weeks and associated with severe  abdominal pain and anaemia with the face pale. In this case you must lie down and immediately call the nearest doctor and inform him that you have
(ECTOPIC PREGNANCY). And you must tell him to bring with him DEXTROSE 5% to fix the drip for you and to trasfer you immediately to the hospital with the drip in your hand.

3. BLEEDING FROM MISSCARRIAGE.

Especially in the first trimester from the time of firtilisation until the 12 weeks. There are many causes some of them due to the UTERUS, some to the BABY, some to the PLACENTA, and some to the HUSBAND, however and whatever you must take it easy knowing that  we are human (one time one apple will drop on the floor before it ripes) but it does not mean that you should neglect the treatment.

4. BLEEDING ANTE-PARTUM.

This bleeding also seriouse usually after 28 weeks of pregnancy.
1. Either because of the placenta implanted low it means  in the lower part of the uterus (LOWER SEGMENT) and near the cervix. This is called PLACENTA PREVIA. Gives bleeding usually without pain may start with little bleeding but aftercan be very severe.

2. Or because the bleeding accumulate behind the placenta with abdominal pan and rigidity of the abdomen, for those two condition you must apply the same EMERGENCY AS I TOLD YOU FOR
ECTOPIC PREGNANCY.

   ABDOMINAL PAIN:

1. psychological point of you almost the same problem of vaginal bleeding, I would like to emphasize that you shoul take it easy and relax so I am going to put these condition in a very easy way.

1. COUITUS.
You get some time very little lower abdominal pain and afterfew minutes relaxation your pain will disappear.
2. Misscarriage.
I have already discussed above in vaginal bleeding.

5. URINE INFECTION.
Usually infection of the blader, urethra (uereter-kidney) = PYELONEPHRITIS. This can cause acute abdominal pain radiating into the lower part of the abdomen and may be associated with haematuria (blood in the urine), vomiting, bacache and sometimes temperature. 6. APPENDICITIS.
Abdominal pain of maximum tenderness is higher in pregnancy especialy in the wrght side and usually associated with nausea, vomiting, temperature and count of thr white blood cells are more than 10.000 in the control of blood test for this reason very easy to diagnose and easy to remove by very simple operation.
7. BOWELL OBSTRUCTION.
Usually caused by adhesion bands altering in positionwith the rising uterus. Colic, distention, vomiting and diminished or absent bowel sound are sign for operation.
