This is a Bradycardia at 55/min. Note the non-capturing pacemaker spikes at a rate of 120/min. I would really like to see some other leads of this and also a tracing in diagnostic mode.
I just received an email from Peter Bonadonna EMT-P (Director of Paramedic Education; Monroe Community College, Rochester, New York.) Thank you Peter, I stand corrected.
"This is a sinus tachycardia at 108 with a normally functioning DDD pacemaker
that is working in the VAT mode (Ventricular paced when triggered by the
atria). Unfortunately due to a long QT interval every other pacer spike
finds ventricular myocardium still repolarizing and unable to respond the
spike. Several things can prolong the QT such as ischemia or hypocalcemia.
While the pacer could be adjusted it is better to find the underlying cause
of the QT prolongation and fixing that." He also sent a Laddergram to demonstrate his diagnosis.
