Lead II
This is a sinus rhythm with complete heart block and a rate of 51/min. Pt was a 76 year-old female who was dig-toxic. Don't be fooled by what appears to be conducted P-Waves. (This from the transferring Internist.) Note the one ventricular escape beat (PVC?)

I am finding myself in agreement with Peter Bonadonna EMT-P from New York. In all likelihood this is probably a Sinus Bradycardia with one PVC. He kindly sent a laddergram to demonstrate his interpretation. Thanks Peter.
