I'm really fairly certain of most of this one. I think the
verses are pretty accurate, but the Em-Bm stuff in the chorus
I'm not totally confident of. And the last few chords at the
end are a little dodgy too. However, I'll put this up for now
and fix it when I figure them out.

Update:
Chris Allumi suggests "What do you think about the chorus possibly
being D Em G F#m?  That seems to be the notes that the bass is
playing.  It also sounds like that's the progression at the end with
A replacing the F#m on the last repeat." I think it might be what
the bass is playing but neither version sounds 100% correct to my
ears.
