Policies
Delivery:
* Customers with accounts in good standing will receive laundry bags and a minimum of 36 regular or 54 infant cloth diapers on the first delivery
* Fresh, clean cloth diapers will be delivered each week to replace the number of soiled diapers placed in the laundry bag outside for pickup.
Pickup:
* Please place your laundry bag of soiled diapers outside in an easily accessible place no later than 8:00 a.m. on the morning of scheduled pickup.
* On the outside of the laundry bag, please note the number of soiled diapers inside.
* If diapers are not available for our driver on your scheduled pickup day, you will be charged your weekly amount. There will be an additional fee for extra pickups.
* If you need additional diapers, please call us to make arrangements. They will be delivered on your next scheduled pickup. If diapers are needed sooner, a delivery fee may apply.
* Please do not place your bag of soiled diapers in the direct sunlight.
* There will be a charge of $2.50 per damaged diaper if diapers are damaged by overexposure to heat and sunlight or overdue cleaning due to missed pickups.
Diaper use, misuse, and abuse:
* Our rental diapers are only to be used on babies only. Diapers that are found to have been used as cleaning rags or for other non baby related uses will need to be removed from service. You will be charged a $3.00 fee per diaper that needs to be removed from service. Using the diaper for baby spit up is acceptable.
* We will take care of the laundering of diapers, please do not soak or treat the diapers in any way. If a diaper is found to have been chemically treated, it will need to be removed from service. You will be charged a $3.00 fee per diaper that needs to be removed from service.
Additional supplies and purchases:
* Our drivers do not carry cash or carry inventory for purchase. Please contact us prior to delivery for supplies. Our drivers will deliver your preordered supplies on your next scheduled pickup date..
Lost and found:
* Please keep all personal items out of the laundry bags. Brian Lewis' Diaper Duty is not responsible for any lost items left in the soiled diaper bag
* Please call us to report any missing items. If found, we will gladly return found items on your next scheduled pickup date.
* Cleaning of lost items may result in a service fee.
Account status and Payment terms:
* Brian Lewis' Diaper Duty provides a rental of a minimum of 36 regular or 54 infant diapers weekly, laundering of soiled diapers for a minimum of $17.92 paid weekly. 6% Florida Sales Tax must be applied.
* All fees must be paid in advance prior to first delivery.
* Deposit of $40.00 is required prior to first delivery. This refundable desposit will be returned to you once service is discontinued and all supplies have been returned to Brian Lewis' Diaper Duty.
* Service fees must be paid in full weekly on the scheduled pickup date. Payment may be in the form of cash ,check, or money order.
* Returned checks will incur a $25 service charge. Service will be suspended until the account is cleared of all balances and fees. If a check is returned, the custmer may be required to pay in form of money order or cash only.
* Brian Lewis' Diaper Duty reserves the right to terminate any account not in good standing.
Vacations
* If you are going on vacation and need a suspension of service, you must call our office at least one week prior to suspension of service. We will discuss arrangements at that time. Additional fees for extra pickup may apply.
Termination of Service
* Customer must provide two weeks notice for termination of service. If proper notice is not given, delivery of diapers will cease, but customer will be billed for the two weeks.
* Brian Lewis' Diaper Duty reserves the right to terminate this agreement at any time.
Arbitration:
You agree that any actions to enforce the terms and conditions for this agreement or for the breach of this agreement shall be referred to binding and final arbitration. Any arbitration proceeding will be governed by the rules, procedures, and protocols of the American Arbitration Association and by the Federal Arbitration Act. The parties hereto expressly waive their rights, if any, to have such matters heard by court or jury, or administrative agency whether federal or state. The arbitrator will be selected by Brian Lewis' Diaper Duty. You agree to pay all fees for arbitration.
Agreement:
I have read the service policies and customer agreement and I agree to these terms and conditions.
