the support and help Dusty is and has been for Sayde) August 1, 2001
Dear Dr.
RE: CHARLEBOIS, Sayde
Date Seen:
I saw Sayde with her mother and aunt in follow-up of her complex developmental disabilities and epilepsy.
Sayde continues on Lamictal mg daily and Valproate mg daily, and her seizures have been relatively stable.
Sayde does have both drop seizures and small myoclonic seizures, with an occasional generalized tonic-clonic
seizure. The small seizures have been more frequent lately, but do not appear to dramatically impair her
functioning. would therefore not suggest any change in her anticonvulsant at the current time Sayde
continues to be on a home-school program, with support from her mother, aunt, and her service dog.
There remains an issue of the acceptability of this service dog to the education system, and the family
continues to struggle with this. Sayde has improved her functioning and is appearing to learn academic
material with the support she is currently receiving. She certainly benefits from the support of her dog.
Thank you for the opportunity of following Sayde. I am pleased that she has been relatively stable.
Sincerely
