3rd cranial (oculomotor) nerve palsy [pupil dilated in complete 3rd cranial nerve palsy] myasthenia gravis [ptosis may be unilateral or asymmetrical] |

rarer causes: |
congenital ptosis ocular myasthenia syphilis muscular dystrophy, eg. facioscapulohumeral, dystrophia myotonica, progressive external ophthalmoplegia |
