thalassaemia [MCV disproportionately lower than Hgb; near-normal Hgb in thalassaemia trait]

congenital sideroblastic anaemia [high Fe and ferritin]

chronic disease [MCV normal or low]

aluminium toxicity [usually on long-term haemodialysis]
