fibrous cortical defect

eosinophilic granuloma [Langerhans cell histiocytosis]

bone tumours, eg. osteoclastoma [NB: bone tumours are usually sclerotic]

'Brown tumour' (in 1° hyperparathyroidism; 'osteitis fibrosa cystica')

osteomyelitis, including tuberculous and Brodie's abscess [chronic indolent infection]

Paget's disease (osteoporosis circumscripta; ± sclerotic areas)
